I have a Yuba Mundo (yubaride.com) that I absolutely love -- it's cheaper than a Surly Big Dummy, but a lot more stable and able to carry weight than an Xtracycle conversion (which has an Xtracycle bolted onto a regular bike frame). The Yuba Mundo genuinely feels like a regular bike and I pass other cyclists regularly, even when carrying a toddler and lots of cargo. Bakfiests seem rad, but they're definitely more suited to flat cities as well as being pretty pricey.
