I had that same pronged retainer. Didn't work, I bent it apart to make my thumb fit. I had the yucky tasting nail polish, too. Even when I was old enough to WANT to stop, it was very difficult to quit. I wish you luck, but I think you will have a very difficult time.

I got a Nikon Coolpix s9100 recently, and it has been a wonderful camera so far. Great zoom (for sneaking up on your child before they notice you taking the picture!) and clear, beautiful photos. I also like the speed with which it turns on initially and that it's ready to take the next picture so much faster than many other digitals I've owned.

Sand is easily covered with a mesh tarp or shadecloth (that way you avoid standing rainwater).

You could create some neat "curtains" with inexpensive shower curtains.

You could buy a small square of artificial grass to set on the bottom, that way you'd have a nice place for them to sit and pretend.

With a more friendly ladder (gentler angle, handles to hang onto) they could possibly be just a few months from handling the ladder with some spotting. I have seen MOST 20 month olds head for and conquer a non-upright ladder on a playstructure (in my experience: 10+ years in child care)

Ikea Expedit is two-sided, and sturdier than a Billy with no back. You can buy a counter/table top and legs at Ikea, as well as the paper roll and roller. You could also create the paper roller with window curtain hardware.

I'm a preschool teacher, and yes, I would appreciate anything with thought and sentiment behind it. A notecard with a heartfelt thank you for what I do always makes my day! As I work hard to make sure the child takes home something special and child-made as a gift for the parents, and also send home something from me to the family...it doesn't seem wrong to hope that each family also thinks of me. It doesn't have to be expensive, but something that shows they thought of me is always appreciated! Starbucks or Target gift cards are a staple, or amazon.com maybe?
