Is it possible that your sister isn't a thoughtless list-maker and instead is overwhelmed by the toys and STUFF that her kids have already? People often indulge children with THINGS and then ask them to be grateful.
My brother and sister-in-law always give experiences for their birthdays or Christmas which has been AMAZINGLY perfect for my kids... They usually provide some kind of gift card (movie tickets, pottery painting gift certificate, manicure gift card, etc) and they plan a day out with each of the kids to redeem it. Consider giving your time OR, if you aren't local, maybe a museum or zoo membership? They'll think of you every time they use it!

We moved both kids to twin beds with low profile frames and side rails at 12 months and had no trouble. Neither could open their bedroom doors so wandering the house and baby-proofing wasn't an issue. I'd say that unless you're going to keep the first one in the crib for a LONG time or have a ton of disposable income, don't waste the money on a second crib.

My husband and I are both fairly tall and didn't even think to look for a crib better suited to our heights. While it may have been nice for the first few months, it may have created some pretty substantial hazards when our son started climbing out of the (fully lowered) crib at 10 months. (Our daughter started climbing out around the same age a couple of years earlier.)

We use a twin duvet w/ cover (which I made) for both kids. No top sheet. My children both thrash around at night;We haven't had a problem with the duvet bunching up in the cover. They wash/dry extremely well every few days.
In the Winter, I add a comforter and top sheet under the duvet.
